Our programs

Care where older adults live

HCA focuses on clinical access and everyday wellbeing for older adults in Sierra Leone, from checkups and disease monitoring to home visits and nutritional support.

Clinical access and community wellness

Our work centres on geriatric clinical access: regular checkups, monitoring of non-communicable diseases such as hypertension and diabetes, and referrals when specialist care is needed.

Alongside clinics we support daily wellbeing, so that older people can keep their independence and remain connected to their families and neighbourhoods.

Core programs

Three ways we reach elders

Mobile health clinics

Community clinics bring screening, checkups and medication review closer to elders who cannot easily travel to a health centre.

Home visits

Trained health workers visit homes to check on mobility, medication and wellbeing, and to support families caring for older relatives.

Nutritional support

Food support and nutrition guidance help elders manage chronic conditions and maintain their strength through the year.

From referral to follow-up

How an elder joins a program

Each enrolled elder moves through the same steps, with community health workers keeping track of progress.

01

Community referral

Neighbours, families or local leaders refer an older person who needs care.

02

Screening

A health worker records blood pressure, blood sugar and mobility at a clinic or home visit.

03

Care plan

Clinical checkups, medication and nutrition are agreed with the elder and their family.

04

Ongoing follow-up

Regular visits track progress, and referrals are made when specialist care is needed.
